PARTICIPANT RELEASE OF LIABILITY. 

____ I understand and agree that the risk of injury involved in this program exists, and while rules, equipment and personal discipline may reduce this risk, the risk of serious injury or death exists and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, even if arising from negligence of the releases of others and assume full responsibility for the registered participant above. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard for the registered participant above, I will remove participant from all such activities and will bring issue to the attention of the nearest staff member. Therefore, I, for myself and on behalf of the participant registered above and any and all personal representatives hereby RELEASE, INDEMNIFY AND HOLD HARMLESS GODINGS FC/KATY, its officers, officials, agents and employees, other participants, sponsoring agencies, advertisers, and if applicable, owners and lessors of premises used to conduct the event, with respect to ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, whether arising from negligence or otherwise, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND THE TERMS AND SIGN IT FREELY AND WILLFULLY.
