Waiver

I understand and agree that the risk of injury involved in this program exists, and while rules, equipment and personal discipline may reduce this risk, the risk of serious injury or death exists. I knowingly and freely assume all such risks, even if arising from negligence of the releasees or others, and assume full responsibility for the registered participant.

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard for the registered participant, I will remove the participant from all such activities and bring the issue to the attention of the nearest staff member.

Therefore, I, for myself and on behalf of the participant registered above and any and all personal representatives, release, indemnify and hold harmless Alexander Perafan Technical Training, its officers, officials, agents and employees, other participants, sponsoring agencies, advertisers, and if applicable, owners and lessors of premises used to conduct the event, with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from negligence or otherwise, to the fullest extent permitted by law.

I have read this release of liability and assumption of risk agreement, fully understand the terms, and sign it freely and willfully.

For parents or guardians of participants under 18 years old: as parent or guardian with legal responsibility for the participant, I have read this release of liability and assumption of risk agreement, fully understand the terms, and sign it freely and willfully.
